From NY state: If someone is missing their vaccination card and cannot go back to the facility they received it at (ie jails or mass pop up vaccination sites), they can email NYS through nysiis@health.ny.gov. A full name and a date of birth will be able to generate an immunization record. It will have the COVID vaccination on it. They cannot get an actual replacement COVID card, however.
